Description
Spironol
tablets – 25 mg
Spironol is a prescription drug containing spironolactone, which belongs to a group of drugs called diuretics (diuretics). The indication for the drug product is the treatment of congestive heart failure, liver cirrhosis with ascites and edema, ascites in the course of malignant neoplasm, nephrotic syndrome, diagnosis and treatment of primary hyperaldosteronism and hypertension.
How to dose Spironol?
The dosage of the product depends on the type of disease and the individual characteristics of the patient. Below are dosage recommendations for specific conditions:
Congestive heart failure with edema:
Initially, usually 100 mg per day in a single dose or in several divided doses.
The dose range may be 25-200 mg per day.
The doctor will determine the maintenance dose individually.
Severe heart failure (NYHA class III-IV):
If the potassium level is not more than 5 mmol/L and the creatinine level is not more than 2.5 mg/dL, the starting dose is 25 mg per day, in combination with standard therapy.
If clinically indicated and well tolerated, the doctor may increase the dose to 50 mg once daily.
If intolerance to a dose of 25 mg 1 time per day persists, the doctor will reduce the dose to 25 mg every 2nd day.
Ascites and edema in cirrhosis:
Initially 100 mg per day or 200-400 mg per day depending on the ratio of sodium ions to potassium ions.
The maintenance dose will be individually determined by the doctor.
Ascites in the course of malignant neoplasm:
Initially, usually 100-200 mg per day.
In severe cases, the doctor will gradually increase the dose to 400 mg per day.
Thereafter, the doctor will adjust the dosage individually.
Nephrotic syndrome (when glucocorticosteroids are ineffective):
100-200 mg per day is taken.
Diagnosis of primary hyperaldosteronism:
Long-term test: 400 mg per day for 3-4 weeks.
If there is an increase in blood potassium concentration (equalization of hypokalemia) and a decrease in blood pressure, the test result is considered positive.
Short-term test: 400 mg per day for 4 days.
If the concentration of potassium in the blood increases during the use of spironolactone and then decreases after its withdrawal, the test result is considered positive.
Primary hyperaldosteronism:
In preparation for surgery, 100-400 mg per day.
The drug can also be used long-term in patients in whom surgery is not possible.
The doctor will individually determine the lowest effective dose for each patient.
Hypertension:
The dose is 25-100 mg per day.
Development of the full antihypertensive effect of the preparation usually requires about 2 weeks of its use.
Before starting the preparation in hypertensive patients taking ACE inhibitors or angiotensin receptor antagonists, blood levels of potassium and creatinine should be determined. If the potassium concentration is greater than 5 mmol/l or the creatinine concentration is greater than 2.5 mg/dl, the preparation should not be started. During the first 3 months of treatment, frequent monitoring of blood potassium and creatinine concentrations is necessary. Thereafter, potassium and creatinine concentrations should be determined every 3-6 months. If the above-mentioned values of potassium and creatinine concentrations are exceeded, the doctor will recommend temporarily stopping or discontinuing the combination treatment.
